Please complete a new Acknowledgement Form for each player and each rostered staff member, ie. 2 Oro players (atom and bantam) and Dad is a trainer on the bantam team means that 3 separate Acknowledgements must be completed.
Failure to complete the Rowans Law Acknowledgement will mean that the player/team staff member will not be rostered and will not be allowed to participate in hockey activities.
